Quando si parla di ginecomastia si intende l’allargamento del tessuto ghiandolare della mammella nell’uomo, ovvero un aumento del volume del seno, intorno al capezzolo ed è un disturbo più frequente di quanto si creda, soprattutto durante l’infanzia e la pubertà, oltre che negli uomini maturi. Non va confusa con la lipomasia, più nota come pseudo ginecomastia, che consiste in un accumulo di grasso all’altezza del seno, in uomini decisamente sovrappeso o obesi. La ginecomastia è la motivazione più frequente per cui gli uomini si sottopongono ad una valutazione medica del proprio seno. Scopriamone insieme le cause, i sintomi correlati e cosa è possibile o necessario fare.
Quali sono le cause ginecomastia?
La ginecomastia è il risultato di uno squilibrio ormonale in cui i livelli di estrogeni (ormoni femminili) aumentano troppo rispetto ai livelli di androgeni (ormoni maschili). Per questo si manifesta nei periodi di sviluppo infantile e puberale (oltre che in prossimità di quella che viene definita andropausa) . Questa ginecomastia fisiologica dei ragazzi di solito scompare da sola entro sei mesi o al massimo due anni. Di rado dura di più e si parla di ginecomastia puberale persistente. Oltre ciò va sottolineato che un ingrossamento del seno nell’uomo può essere la conseguenza di alcune problematiche di salute che possono alterare i livelli ormonali, come le seguenti:
- Malnutrizione e ri-alimentazione (recupero dalla malnutrizione)
- Cirrosi epatica
- Disturbi degli organi sessuali maschili (testicoli) e correlato calo di testosterone
- Tumore ai testicoli
- Insufficienza renale cronica
- Ipertiroidismo
- Utilizzo di alcuni farmaci
Sintomi correlati alla ginecomastia
Il principale della ginecomastia è l’aumento del volume del seno maschile. In genere il disturbo è simmetrico, cioè si manifesta su entrambe le mammelle anche se non mancano casi in cui è unilaterale o accentuato da una parte ed è comunque morbido al tatto. Può essere presente una particolare sensibilità, ma non dolore. In queste caratteristiche si distingue dal poco noto tumore al seno dell’uomo, che abitualmente riguarda una sola mammella, presenta un rigonfiamento duro, non necessariamente intorno al capezzolo, può essere associato a fossette della pelle, retrazione e/o secrezione dal capezzolo, linfonodi gonfi.
Ginecomastia, quali possibilità di cura?
La ginecomastia puberale spesso va via da sola in circa sei mesi, senza necessità di alcun trattamento, anche se è sempre importante tenere la situazione sotto osservazione. Per tutte le altre cause è prima di tutto importante identificarle e curarle: anche sospendere laddove possibile e dietro consulenza medica, l’utilizzo di quei farmaci che possono averla determinata. Esistono anche dei medicinali atti a curare direttamente la ginecomastia, benché nessuno ancora autorizzato con questa indicazione precisa perché i dati sull’efficacia non sono ancora certi. Le terapie finora adottate prevedono:
- Terapia ormonale sostitutiva con testosterone (per gli anziani con bassi livelli di testosterone)
- Clomifene
- Tamoxifene
- Danazolo
Tutti questi rimedi farmacologici si sono rilevati efficaci solo nel trattare la ginecomastia nelle fasi iniziali, ma dopo circa un anno si creano degli esiti cicatriziali nel tessuto mammario, impossibili da eliminare se non attraverso la chirurgia: la mastoplastica riduttiva (chirurgia di riduzione del seno) è abitualmente utilizzata nei casi più gravi.
