Le malattie sessualmente trasmissibili sono infezioni che, come suggerisce il nome, si trasmettono attraverso un rapporto sessuale. Di qualunque tipo, sia vaginale che anale o orale. Possono provocare sintomi molto fastidiosi o addirittura gravi, oppure non arrecare disturbi di alcun tipo, alcune sono curabili ed altre purtroppo no. Malattie sessualmente trasmissibili, cause e quali sono
Queste patologie possono essere provocate da diversi tipi di microrganismi (virus, batteri, parassiti, funghi). Tra le infezioni virali troviamo:
- Papillomavirus umano (HPV): si sta pensando anche ad una vaccinazione al maschile così come avviene per le giovani donne per evitare lo sviluppo di complicanze
- HIV o virus dell’immunodeficienza umana
- HSV o virus dell’Herpes simplex
- Epatiti
Infezioni batteriche sessualmente trasmissibili includono la sifilide, la gonorrea, e la clamidia.
La tricomoniasi è invece provocata dal parassita Trichomonas, così come i pidocchi e la scabbia che pure si possono trasmettere durante un rapporto sessuale.
I sintomi delle malattie sessualmente trasmissibili negli uomini
Essendo diversi gli organi riproduttivi degli uomini rispetto alle donne, i sintomi di queste patologie possono essere differenti e non sempre sono presenti in ambedue i generi. Negli uomini, le malattie sessualmente trasmissibili possono essere raggruppate in tre categorie ben distinte in base proprio alla sintomatologia:
- MST che causano lesioni genitali (herpes )
- MST che possono provocare l’infiammazione dell’uretra (uretrite– esempio con la clamidia o la gonorrea)
- MST che causano sintomi e segni in tutto il corpo (MST sistemica)
Alcune delle malattie sessualmente trasmissibili che causano lesioni o uretrite locali, tra cui la gonorrea e la sifilide, possono anche causare danni ad altri organi e diffondersi all’interno del corpo, se non trattati. In presenza di uretrite, l’infiammazione è spesso accompagnata da infezioni delle vie urinarie (correlate a bruciore e dolore durante l’emissione di urina o scarico dall’uretra).
A seconda del tipo di infezione, le lesioni possono svilupparsi come verruche genitali, vesciche dolorose o vere e proprie ulcere.La gonorrea può colpire anche la gola ed il retto e diffondersi al resto del corpo con eruzioni cutanee e dolori articolari. Per ciò che riguarda l’HIV, sul lungo termine (10 anni) si sviluppano sintomi legati ad immunodeficienza, ma inizialmente non vi sono segni specifici, anche se si possono sviluppare entro il mese dal contagio sintomi similinfluenzali (che passano solitamente inosservati.
Le lesioni da herpes simplex (HSV) provocano piaghe e vesciche dolorose sulle zone sessualmente esposte del corpo in entrambe i sessi. Negli uomini, tali lesioni si possono sviluppare su pene, scroto, glutei, ano, dentro l’uretra o sulla pelle delle cosce. Il primo focolaio di infezione da HSV può essere più grave dei focolai successivi e può essere accompagnato da febbre e gonfiore dei linfonodi.
Diagnosi
La maggior parte delle malattie sessualmente trasmissibili sono diagnosticate sulla base della storia clinica del paziente e da reperti organici, va cioè identificato il microrganismo infettivo. Per farlo esistono analisi del sangue e test specifici.
Quale cura?
La terapia come sempre in questi casi dipende dalla causa: clamidia, gonorrea e sifilide, sono in genere curabili con antibiotici. L’infezione da Trichomonas può anche essere curata con farmaci antiparassitari. Per le infezioni di origine virale, non ci sono grandi possibilità di cura: alcune come l’HPV nella maggioranza dei casi si risolvono da sole grazie all’impegno del sistema immunitario, le verruche possono essere rimosse, per l’epatite sono utili interferone ed antivirali. Gli antivirali sono usati anche per l’HIV, ma stiamo parlando di condizioni croniche, impossibili da debellare, bensì da tenere sotto controllo il più a lungo possibile.
