Lo stress è stato lungamente incriminato come una delle principali cause di aborto spontaneo precoce. Al riguardo però ancora non esistono studi scientifici a confermare in modo definitivo questa ipotesi: la letteratura al riguardo è scarsa e a volte anche contraddittoria. Ma allora quale è la verità? E quali sono le cause più comuni dell’aborto spontaneo?
Cause di aborto spontaneo
Circa il 20 % delle gravidanze finisce precocemente (entro le 12 settimane) con un aborto spontaneo: la causa più frequente è da riscontrare in un’anomalia cromosomica o in un altro problema di sviluppo dell’embrione/ feto. A questi si aggiungono poi malformazioni o altre complicanze dell’utero, reazioni immunitarie che agiscono contro l’impianto, squilibri ormonali, disturbi di coagulazione del sangue, patologie croniche e altri fattori di rischio noti per l’aborto spontaneo come l’assunzione di sostanze alcoliche, il tabagismo e l’esposizione ad altre sostanze.
Stress ed aborto spontaneo
Come è noto una reazione emotiva molto forte, come uno shock luttuoso ad esempio, può indurre alterazioni ormonali tali da rendere complicato anche lo stesso impianto dell’embrione (pensiamo ai casi di amenorrea da stress emotivo). Un’ incidenza c’è dunque a livello preconcezionale, di difficoltà a rimanere in stato interessante e allo stesso modo anche quindi, nelle primissime settimane di gestazione a mantenere stabile l’embrione nell’utero. A conferma di ciò alcuni studi più recenti frutto di una nuova branca medica come la psiconeuroendocrinoimmunologia. Di particolare interesse una ricerca condotta in Israele e pubblicata su Psychosomatic Medicine nel corso della quale sono stati confrontati i numeri di aborto spontaneo tra due gruppi di donne (nel periodo 2004-2008) afferenti allo stesso nosocomio, ma provenienti da zone diverse: un gruppo dalla striscia di Gaza (quindi emotivamente sempre sotto stress) e le altre da un’area più tranquilla. Ebbene nelle prime il tasso di abortività è stato superiore del 50% rispetto alle altre, fatta esclusione di anomalie fisiche.
Tutta colpa di un ormone
Tali dati sono riconducibili al ruolo di un particolare ormone secreto dall’ipotalamo in abbondanza durante una fase di stress: il Crh (corticotropin releasing hormone). Questo interferisce sugli ormoni sessuali e riproduttivi e dunque con i meccanismi di impianto, stabilizzazione e sviluppo dell’ovulo fecondato.
Stress ed aborti ricorrenti
Di contro, dopo aver perso un bimbo si instaura nella donna un’ansia fisiologica che può sfociare anche in depressione. E’ paura, sensazione di incapacità, di inutilità. Per tale motivo è spesso necessario rivolgersi a gruppi di auto aiuto, supporters, psicologi e psicoterapeuti soprattutto se si è vissuta questa esperienza negativa ripetutamente (si parla di aborto spontaneo ricorrente quando questo evento si manifesta due o più volte consecutivamente). E spesso questo stress che ne deriva, se non affrontato blocca la capacità di portare avanti una gravidanza (per i motivi di cui sopra) laddove non esistano e siano state indagate cause fisiche. Difficilmente però aborti ricorrenti sono dovuti ad uno stress di base: solitamente è un problema organico a provocarli.
