A cosa serve il cortisone? E quali effetti collaterali può avere? I corticosteroidi (ormoni) sono farmaci importanti, utili in numerose patologie in quanto capaci di ridurre o addirittura bloccare la risposta dei tessuti alle infiammazioni: riducono il gonfiore, l’infiammazione, il dolore in patologie come l’artrite, l’asma, la psoriasi, la sclerosi multipla….Bentelan, deltacortene, prednisolone, eccetera, ne sono degli esempi utilizzati comunemente. Come tutti i farmaci ovviamente anche il cortisone può avere effetti collaterali a breve o lungo termine a seconda dei casi. Analizziamoli insieme concentrandoci su quelli di uso più comune negli adulti, ovvero la formulazione in compresse, per via orale.

Come si usa, dosi e precauzioni

E’ preferibile assumere cortisone a stomaco pieno, o comunque durante i pasti onde evitare effetti diretti sulle pareti dello stomaco. Di solito si assume con acqua. Il dosaggio e la durata del trattamento dipendono dal problema di salute che si deve curare ed è opportuno attenersi alla prescrizione medica (necessaria per l’acquisto anche di una sola confezione di Bentelan). Occorre proseguire la terapia fino alla fine e non interromperla se ci si sente meglio e senza aver consultato il proprio medico curante a cui andranno riferiti anche tutti gli eventuali effetti collaterali. Inoltre è preferibile scalare il dosaggio con gradualità per la sua sospensione.

 

Effetti collaterali comuni del cortisone

Gli effetti collaterali più comuni del cortisone in compresse (a questa formulazione ci riferiamo anche a seguire) raramente sono pericolosi e si manifestano comunque sul breve termine. Tra questi troviamo:

 

  • Lieve stato confusionale
  • Anomalo senso di euforia, eccitazione, irrequietezza
  • Mal di testa
  • Nausea
  • Vomito
  • Problemi cutanei: acne, rossore, ipersudorazione e, sul lungo termine assottigliamento della pelle; in caso di assunzione (o applicazione topica) è sconsigliato esporsi al sole per evitare le macchie solari
  • Disturbi del sonno
  • Aumento dell’appetito e del peso

Il più delle volte tali effetti avversi sono di lieve entità e tendono a scomparire da soli in pochi giorni o settimane. Più raramente sono imponenti, gravi o vanno via con difficoltà: in tutti i casi è necessario parlarne con il proprio medico curante per stabilire l’eventuale proseguo della cura in base al rischio/beneficio ed aggiustare eventualmente il dosaggio.

 

Effetti collaterali gravi del cortisone

Il cortisone può provocare, seppur raramente, degli effetti collaterali pericolosi, che comportano un’emergenza medica. Chiamare immediatamente il medico ed il 118 se ci si trova in una delle seguenti situazioni:

  • Reazione allergica (tra i sintomi: eruzione cutanea, orticaria, bruciore, gonfiore del viso, delle labbra, della gola o della lingua)
  • Squilibri elettrolitici (tra i sintomi: ritenzione idrica)
  • Insufficienza cardiaca (fiato corto, tachicardia, arti gonfi, pressione alta)
  • Problemi muscolari (debolezza)
  • Osteoporosi e conseguenti fratture
  • Debolezza e rottura delle aricolazioni
  • Problemi allo stomaco, come l’ulcera peptica, sangue nelle feci o feci scure
  • Pancreatite (mal di stomaco, nausea, vomito)
  • Glaucoma (visione  sfocata, doppia)
  • Convulsioni
  • Aumento / diminuzione della motilità e numero di spermatozoi negli uomini
  • Irregolarità mestruali nelle donne

 

Alcune di queste reazioni si manifestano sul lungo termine (come l’osteoporosi) in conseguenza di una terapia per una malattia cronica. Per ovviare al problema o ritardarlo al massimo il medico prescrittore potrà voler valutare lo stato generale delle condizioni di salute del paziente ed eventualente aggiungere degli aggiustamenti (con integratori o altri medicinali).

Effetti collaterali cortisone nei bambini

Oltre ai problemi sopra descritti per gli adulti, nei bambini che fanno un utilizzo prolungato di cortisone è possibile che subentrino problemi di crescita rallentata.
