Cos’è la stenosi carotidea? Forse una delle malattie cardiovascolari più trascurata dagli individui, nonostante la sua frequenza specie in fasce d’età avanzate: si usa questo termine -“stenosi”- per indicare un restringimento della carotide. In genere è provocato dall’accumulo di sostanze grasse, noto come placca. Maggiore è la dimensione della placca e più aumenta il rischio per la vita. In particolare quando le carotide sono ostruite si eleva in modo esponenziale il rischio di ictus.

Le arterie carotidi

Le arterie carotidi sono due grandi vasi sanguigni poste sui lati del collo. Sono fondamentali per apportare sangue ossigenato alla parte anteriore del cervello, quella dove risiedono il pensiero, la capacità di parlare, le caratteristiche della personalità, oltre che le funzioni sensoriali e motorie. E’ chiaro dunque come e perché sia particolarmente importante che le carotidi siano in grado di far fluire sangue ossigenato. Attraverso le carotidi si possono anche sentire le pulsazioni cardiache.

Cause della stenosi della carotide

Come le arterie che forniscono sangue al cuore – le coronarie – quelle carotidee possono anche sviluppare aterosclerosi.
Nel corso del tempo, l’accumulo di sostanze grasse e colesterolo restringe questi vasi, dando origine ad una diminuzione del flusso di sangue al cervello, aumentando come dicevamo, il rischio di ictus.
Questo si può manifestare per vari motivi, ma in relazione alla stenosi carotidea può svilupparsi quando:
• L’arteria si restringe in maniera eccessiva
• Un pezzo di placca si stacca e viaggia fino alle piccole arterie cerebrali
• Si forma un coagulo di sangue e ostacola un vaso sanguigno
I fattori di rischio per la malattia dell’arteria carotidea sono simili a quelli di altri tipi di malattie cardiache. Essi comprendono:
• Età
• Fumo
• Ipertensione (pressione alta) – il fattore di rischio più importante curabile per l’ictus
• Eccesso di lipidi o colesterolo alto
• Insulino-resistenza
• Diabete
• Obesità
• Stile di vita sedentario
• Storia familiare di aterosclerosi, malattia coronarica o malattia dell’arteria carotidea

 

Sintomi di stenosi della carotide

La stenosi carotidea può non comportare sintomi particolari. In genere infatti si scopre con dei controlli di routine (che andrebbero sempre fatti dopo i 45 anni anche e soprattutto per questi motivi), o peggio, quando è in una fase abbastanza avanzata da privare il cervello dell’afflusso di sangue e quindi provocare un ictus o una TIA (Attacco Ischemico Transitorio).

Sintomi di ictus eTIA

• Improvvisa visione offuscata, o difficoltà a vedere da uno o entrambi gli occhi
• Debolezza, formicolio, intorpidimento o su un lato della faccia, del corpo, o in un braccio o una gamba
• Difficoltà improvvisa nel camminare, perdita di equilibrio, mancanza di coordinamento
• Vertigini e / o stato confusionale
• Difficoltà a parlare (afasia)
• Confusione
• Mal di testa improvviso e violento
• Problemi con la memoria
• Difficoltà di deglutizione (disfagia)

La differenza tra TIA ed ictus è che la prima ha una breve durata dei sintomi (pochi minuti o poche ore), ma entrambe sono emergenze mediche: è impossibile sapere in cosa evolverà la situazione.
Per questi motivi è importante parlare con il proprio medico curante se si ha familiarità con la malattia dell’arteria carotidea o se si hanno i fattori di rischio sopra elencati. Altresì è importante sottoporsi ai check up e alle terapie del caso, sia per la diagnosi che per la prevenzione. Esistono diverse possibilità per curare la stenosi carotidea prima che questa evolva in complicanze pericolose.
