Il gonfiore scrotale è un anormale allargamento dello scroto, la sacca che circonda i testicoli. E’ una problematica che può manifestarsi nei maschi di ogni età, dal neonato all’anziano, può comportare dolore e spesso coinvolge i testicoli (ma non sempre), per questo si parla soprattutto di testicoli gonfi. Può inoltre colpire solo il testicolo destro o quello sinistro o essere bilaterale, a seconda dei casi. Tale condizione si può accompagnare anche ad altri sintomi come la febbre, il sangue nelle urine o la presenza di un grumo al tatto/palpazione. La torsione del testicolo è sicuramente la causa più pericolosa di gonfiore scroto-testicolare. Si tratta di un’emergenza grave, che va trattata prontamente in un pronto soccorso: il testicolo si contorce all’interno dello scroto, bloccando così l’afflusso di sangue. Se questa torsione non viene trattata rapidamente, il testicolo può essere perso definitivamente. Questa condizione è estremamente dolorosa. Per fortuna non si tratta di una problematica frequente, ma comunque non va sottovalutata. Tra le altre cause troviamo:

• Alcuni trattamenti medici
• Insufficienza cardiaca congestizia
• Infezione batterica
• Epididimite
• Linfedema
• Parotite (orecchioni)
• Spermatocele
• Ernia
• Idrocele
• Lesione
• Orchite
• Chirurgia nella zona genitale (esempio dopo operazione per l’ ernia)
• Varicocele
• Il cancro del testicolo

Rimedi e cura per i testicoli gonfi

Se il dolore è molto forte posizionare un asciugamano arrotolato tra le gambe appena sotto lo scroto per contribuire ad alleviarlo e ridurre il gonfiore, ma consultare immediatamente un medico per assicurarsi che non si tratti di una torsione. Soprattutto se riguarda un solo lato, il testicolo destro o il sinistro (la possibilità di una torsione è più alta). Recarsi in un pronto soccorso o chiamare il 118 se non si è in grado di andare o reperire il proprio medico curante. Se invece il dolore correlato al gonfiore dello scroto o dei testicoli è sopportabile si possono intraprendere alcune accortezze in attesa di una visita specialistica o comunque durante la terapia: impacchi di ghiaccio allo scroto per le prime 24 ore per diminuire il gonfiore; indossare un sostenitore atletico loose-fitting per le attività quotidiane ed evitare una eccessiva attività motoria o sportiva fino alla scomparsa dei sintomi. In caso di febbre correlata è possibile assumere un antipiretico. A seconda della causa il medico potrà prescrivere antibiotici e farmaci per il dolore, o raccomandare un intervento chirurgico. Una ecografia scrotale può altrettanto essere consigliata per una diagnosi precisa che ovviamente va effettuata quanto prima, soprattutto se il gonfiore ai testicoli o allo scroto non scompare nell’arco di qualche giorno, anche in assenza di dolore.
