Il prurito al glande è un riscontro che può essere considerato relativamente frequente, dopo un rapporto sessuale non protetto o con preservativo, accompagnato da una patina bianca o macchioline rosse, diffuso a tutto il pene o solo al prepuzio, con gonfiore e/o bruciore. Può essere lieve come pure fortemente invalidante e costante ed accompagnarsi ad altri sintomi che vanno da vesciche e crosticine fino a febbre o mal di gola. Tutto dipende dalle cause che lo hanno provocato, in genere un qualche tipo di infezione, una malattia a trasmissione sessuale o problemi di igiene. Vediamo insieme quali le cause più frequenti ed i possibili rimedi e cure.
Cause di prurito al glande
Molte malattie sessualmente trasmesse possono provocare prurito al pene e nello specifico al glande. Tra queste troviamo più di frequente:
- Chlamydia
- Verruche genitali (papillomavirus umano o HPV)
- Gonorrea
- Herpes
- Mollusco contagioso
- Pidocchi
- Scabbia
- Trichomonas vaginalis
Tra le altre cause non trasmissibili per via sessuale si possono evidenziare:
- Scarsa igiene
- Irritazione da contatto (ad esempio dovuta ad un detergente o ad un suo ingrediente, al detersivo utilizzato per lavare la biancheria intima)
- Indumenti troppo stretti
- Reazioni allergiche
- Balanite (infiammazione del prepuzio)
- Dermatite
- Lichen sclerosus et atrofico (una rara malattia della pelle che sbianca la cute e la rende sottile e a chiazze)
- Psoriasi
- Tinea cruris ( infezione fungina dell’inguine)
- Infezioni del tratto urinario
Come si fa la diagnosi
Il prurito al glande (o comunque al pene) non è considerabile come un’emergenza medica, ma poiché può essere provocato da virus e batteri sessualmente trasmissibili è opportuno astenersi da rapporti non protetti per non contagiare il partner. Al contempo occorre vincere l’imbarazzo e farsi visitare da un medico perché nella stragrande maggioranza dei casi si può e si deve avere una cura, anche e soprattutto per evitare eventuali complicanze che possono essere gravi (difficoltà a ritrarre il prepuzio, infertilità, tumore, cicatrici, diffusione di infezione) sul lungo termine se non trattata la malattia di base. Durante la visita medica -con l’andrologo o il dermatologo esperto in malattie veneree- lo specialista valuterà la cute e la presenza eventuale di altri sintomi oltre il prurito (verruche, crosticine, macchioline rosse, patina bianca, eccetera) che potranno suggerire una causa specifica o escluderne alcune. Possono poi essere necessari alcuni test, come un tampone, per avere la certezza diagnostica.

Quali rimedi e cure per il prurito al glande lo stabilirà il medico specialista una volta fatta la diagnosi. Ad esempio in caso di un’infezione batterica potrà essere necessario applicare pomate antibiotiche o assumere antibiotici per bocca, per funghi saranno utili antimicotici e per irritazioni dovute a sostanze chimiche basterà individuarne le responsabili ed evitarle. L’igiene intima non può mai prescindere.
