Un trapianto di fegato associato ad una derivazione interna dell’intestino ha cambiato la vita al piccolo Francesco (nome inventato), piccolo paziente toscano affetto da colestasi intraepatica familiare progressiva di tipo I (PFIC1). Una malattia molto rara, che colpisce 1 bambino ogni 100.000, e che lo ha costretto a mangiare solo patate nei primi quattro anni di vita.
Ma grazie all’intervento sopra menzionato, Francesco è letteralmente rinato. Dopo un primo trapianto di fegato, due anni fa, che non era andato bene, il piccolo Francesco è stato nuovamente operato. Stavolta, al classico trapianto, i medici IRCCS ISMETT (Istituto Mediterraneo per i Trapianti e Terapie ad Alta Specializzazione) di Palermo hanno operato anche una derivazione interna dell’intestino. E tutto è finalmente cambiato. Il bambino è stato dimesso da ISMETT meno di un mese dopo il doppio intervento. Il prof Jean de Ville de Goyet, a capo dell’équipe che ha eseguito l’intervento, ha dichiarato:
Normalmente i bambini che soffrono di questo tipo di patologia possono beneficiare della derivazione della bile all’esterno, creando una stomia sull’addome. Ovvero viene creata un’apertura per poter mettere in comunicazione l’intestino con l’esterno che scarica all’interno di una sorta di sacchetto. Negli ultimi anni è stato proposto di derivare la bile nel colon, ma questo tipo di operazione viene effettuata a bambini non trapiantati. Nel caso di Francesco, vista anche la sua giovanissima età, abbiamo scelto di realizzare nello stesso tempo questi due interventi essenziali, ovvero agire sul canale della bile al momento del trapianto e fare in modo che la bile scarichi dritto nel colon. Questa operazione combinata consentirà al piccolo di non avere più dissenteria cronica, di tornare ad una dieta normale e contemporaneamente gli ha permesso di non avere nessun drenaggio esterno. Sono certo che questo intervento sicuramente migliorerà la sua qualità di vita
