Ebola cos’è e come avviene la trasmissione (contagio)

Ebola, conosciuta anche come febbre emorragica, è una malattia mortale provocata da un uno dei ceppi del virus Ebola (famiglia Filoviridae, genere Ebolavirus). Non è una novità in termini clinici, si tratta infatti di un tipo di virus identificato nel 1976 nei pressi del fiume Ebola –da cui ha preso il nome- in quella che oggi è la Repubblica Democratica del Congo. Da allora, focolai sono apparsi sporadicamente in Africa. Perché ci spaventa tanto? Non esiste ancora una cura, ha un’alta mortalità (oltre il 50%) ed è difficile prevenire il contagio. Scopriamo meglio come questo avviene.

Contagio: come si trasmette il virus ebola?

Non è stato ancora identificato il serbatoio ospite naturale del virus Ebola: questo significa che non sappiamo ancora come inizia un focolaio (ovvero il primo paziente come si infetta). Tuttavia,si ritiene che la trasmissione avvenga tramite il contatto di con un animale portatore del virus. Una volta che l’infezione ha colpito l’essere umano, inizia a diffondersi in diversi modi: attraverso il contatto diretto (ferite sulla pelle o mucose, per esempio, gli occhi, il naso o la bocca) con:

  • sangue o fluidi corporei (inclusi, ma non limitati a urina, saliva, sudore, feci, vomito, latte materno, e sperma) di una persona malata
  • oggetti (come aghi e siringhe) che sono stati contaminati con il virus
  • animali infetti

E’ per questi motivi che gli operatori sanitari che si occupano di pazienti colpiti da Ebola risultano essere a maggior rischio di ammalarsi: perché possono entrare in contatto con sangue o fluidi corporei infetti di pazienti malati.

Ebola non si diffonde attraverso l’aria o l’acqua, o, più in generale, con il cibo, anche se si “ipotizza”-dunque nessuna certezza- che i focolai africani di questa febbre emorragica possano avere origine dal consumo di carne di animali selvatici (oltre che con il contatto con i pipistrelli infetti). Non vi è alcuna prova che le zanzare o altri insetti possono trasmettere il virus Ebola. Solo i mammiferi (per esempio, gli esseri umani, pipistrelli e primati) hanno dimostrato la capacità di essere infettati con questo virus e diventarne portatori contagiosi.
Il virus Ebola può essere trasmesso anche indirettamente, dal contatto con superfici e oggetti precedentemente contaminati. Il rischio di trasmissione di queste superfici è comunque basso e può essere ridotto ulteriormente da adeguate procedure di pulizia e disinfezione.

 

Quando si è contagiosi?

Questo è un quesito difficile a cui rispondere. Il CDC statunitense afferma che il virus assume la capacità di trasmettersi nel momento in cui il paziente diventa sintomatico, non durante l’incubazione e neppure dopo la guarigione. Il problema è nella definizione di sintomi che come anche per tutte le sindromi influenzali virali possono iniziare il modo aspecifico, con lievi malesseri. Non necessariamente ad esempio con una febbre superiore ai 38,6° C come il protocollo di sicurezza prevede (alcuni esperti suggeriscono una rapida revisione dati gli ultimi contagi extra africani, documentabili con maggiore precisione). In più il virus è stato trovato nello sperma di un paziente guarito dopo 70 giorni, il che lascia pensare che ci sia ancora un rischio di trasmissione per via sessuale fino a tre mesi dopo.

Quali i sintomi di Ebola?

Una diagnosi di Ebola prevede in generale il raggiungimento dei seguenti sintomi : febbre superiore a 38,6 ° C, forte mal di testa e mal di pancia, dolore muscolare diffuso, vomito, diarrea e violente ed inspiegabili emorragie (lividi o sanguinamento vero e proprio).
I sintomi possono apparire 2-21 giorni dopo l’esposizione al virus Ebola, ma la media è di 8 a 10 giorni (da qui si capisce anche che l’incubazione può essere brevissima, anche solo 48 ore). La guarigione dipende dalle cure e dal sistema immunitario del paziente. E’ documentata al momento una sopravvivenza pari al 50%. Chi supera la malattia sembra mantenerne gli anticorpi per almeno 10 anni.
