Cos’è la terapia adiuvante e perché viene somministrata
La terapia adiuvante è una parte fondamentale del trattamento del carcinoma della mammella, destinata a ridurre il rischio di recidiva dopo l’intervento chirurgico. Viene somministrata a completamento della chirurgia, con l’obiettivo di eliminare eventuali cellule tumorali residue che non sono visibili con gli esami diagnostici.
Questa strategia si basa sul principio che, anche in presenza di una rimozione completa del tumore visibile, micrometastasi potrebbero essersi già diffuse nell’organismo. Trattandole precocemente, si riduce in modo significativo la probabilità che la malattia si ripresenti, localmente o a distanza.
Il concetto di “adiuvante” implica un approccio preventivo e di consolidamento, volto a rafforzare l’efficacia della terapia primaria. A seconda del tipo di tumore, delle sue caratteristiche molecolari e dello stato di salute della paziente, si sceglie il trattamento più indicato.
Gli studi clinici hanno dimostrato che la terapia adiuvante è in grado di aumentare la sopravvivenza globale e la sopravvivenza libera da malattia, soprattutto nei casi di tumori aggressivi o a rischio intermedio-alto.
Tipologie di terapia adiuvante alla mammella

Infine, la radioterapia è indicata soprattutto dopo interventi conservativi al seno, per eliminare eventuali cellule tumorali rimaste nell’area operata. Anche in caso di mastectomia, può essere prescritta se il rischio locale è elevato.
Fattori che influenzano la scelta della terapia
La decisione di somministrare una terapia adiuvante, e il tipo di trattamento da adottare, dipendono da una serie di fattori clinici e biologici. Tra i più rilevanti troviamo dimensioni del tumore, grado istologico, stato dei linfonodi, recettori ormonali, proliferazione (Ki-67) e stato HER2.
Anche l’età, le condizioni generali di salute, la menopausa e le preferenze della paziente entrano nel processo decisionale. In oncologia moderna, la tendenza è quella di personalizzare il trattamento, per offrire il massimo beneficio terapeutico con il minimo impatto sulla qualità della vita.
Negli ultimi anni si sono affermati anche test genomici, come Oncotype DX, MammaPrint e EndoPredict, che analizzano l’attività di specifici geni nel tessuto tumorale. La prevenzione delle recidive è uno degli obiettivi principali della terapia adiuvante, ma richiede anche un attento monitoraggio clinico nei mesi e negli anni successivi alle cure. Il follow-up oncologico è studiato per intercettare eventuali segni precoci di ritorno della malattia, garantendo così un trattamento tempestivo.
In genere, il follow-up comprende visite periodiche, esami del sangue, ecografie, mammografie e, quando indicato, imaging avanzato come la risonanza magnetica o la PET-TC. L’intensità dei controlli viene modulata in base al rischio individuale della paziente e alla terapia ricevuta.
