Esso può essere essenzialmente di due tipi, o causato dalla rottura di un vaso sanguigno nel cervello, caso che occorre nel 10 % dei casi circa, oppure a causa della occlusione parziale o totale dei vasi sanguigni, con conseguente riduzione del flusso sanguigno al cervello. E’ questo il caso più frequente di ictus, circa il 90 % dei casi. Nel primo caso si parlerà di emorragia cerebrale, nel secondo di ischemia.
Le cause sono da ricercarsi essenzialmente in problemi dell’apparato circolatorio, dall’ipertensione e dall’arteriosclerosi, che contribuiscono, in modi diversi all’improvvisa occlusione o rottura dei capillari nel cervello.
L’ictus ha la caratteristica di giungere all’improvviso, senza alcuna anticipazione, e tra i sintomi più comuni si ha la parziale o totale paralisi di metà del viso e del corpo, perdita della vista, difficoltà di espressione e di linguaggio, vertigini ed anche perdita totale di coscienza.
Ci possono essere casi di ictus che non provocano alcun sintomo visibile, ed in alcuni casi si ha la cosiddetta ischemia transitoria, un attacco che si evolve e termina nell’arco delle 24 ore, ma che può essere un campanello d’allarme per successivi attacchi più gravi in futuro.
Non ci sono terapie che possono, una volta avviato il processo ischemico risolvere favorevolmente l’attacco, ed in seguito tutto dipende dalla gravità e vastità del danno provocato al tessuto cerebrale dal mancato afflusso di sangue. In alcuni casi, attraverso terapie riabilitative, si po’ recuperare quasi il 90 % delle funzioni che si possedevano precedentemente all’attacco, perchè alcune parti del cervello sono in grado di sostituirsi alle parti danneggiate, mentre in altri casi il danno diventa irreversibile.
La prevenzione è quindi una delle armi migliori per combattere i fattori di rischio di ictus, sopratutto un monitoraggio attento della salute della circolazione arteriosa per evitare l’arteriosclerosi, attuando norme dietetiche e di salute in grado di ridurre la formazione di placche nel tessuto arterioso.
