Le cause di questo male sono molteplici, anche derivate da infezioni virali.
La cardiomiopatia può essere classificata come primitiva o secondaria. Quella primitiva non può essere attribuita ad un evento specifico, come pressione alta, malfunzionamento delle valvole cardiache, problemi alle arterie o difetti cardiaci congeniti, mentre quella secondaria è legata a cause specifiche ed è spesso da associare ad altri disturbi che oltre al cuore coinvolgono altri organi.
Ci sono tre tipi principali di cardiomiopatia: dilatativa, ipertrofica e restrittiva.
La cardiomiopatia dilatativa è la forma più comune: in essa, la cavità del cuore si allarga e si allunga, il cuore diventa debole e non riesce a pompare normalmente il sangue. In questa condizione il paziente sviluppa un’insufficienza cardiaca, e si possono presentare altri disturbi come, per esempio, anomalie nel ritmo del battito cardiaco denominate aritmie.
A causa dell’ingrossamento delle cavità cardiache il sangue scorre più lentamente e ciò può provocare la formazione di coaguli del sangue. Questo può provocare trombosi, quando il grumo di sangue finisce per bloccare un arteria, o embolo quando viene bloccato un capillare.
La cardiomiopatia ipertrofica si presenta quando aumenta la massa muscolare del miocardio, che va ad ingrossare la parte presente tra i due ventricoli ostacolando in questo modo l’afflusso di sangue dal ventricolo sinistro.
Questo disturbo è denominato come cardiomiopatia ipertrofica ostruttiva, ipertrofia asimmetrica del setto oppure stenosi subaortica ipertrofica idiopatica.
Oltre ad ostacolare il flusso di sangue, la parete ispessita a volte distorce una delle ali della valvola mitralica che dunque non riesce più a contenere il flusso del sangue.
La cardiomiopatia ipertrofica è il più comune tra i difetti cardiaci ereditari, statisticamente è presente in una persona su 500 e spesso è asintomatica.
Un’altra forma del disturbo, denominata cardiomiopatia ipertrofica non ostruttiva occorre quando il muscolo ipertrofico non ostacola il flusso di sangue.
I sintomi della cardiomiopatia ipertrofica comprendono dispnea da sforzo, vertigini, svenimenti e angina pectoris, il dolore al torace provocato dal ridotto afflusso di sangue al muscolo cardiaco. In alcuni pazienti si possono verificare aritmie cardiache. L’ostruzione del flusso di sangue nel ventricolo sinistro aumenta il lavoro di questo, il che causa il cosiddetto soffio al cuore.
La cardiomiopatia restrittiva è il disturbo meno comune, e si manifesta quando il miocardio diventa rigido, rendendo quindi più difficile per i ventricoli riempirsi di sangue tra un battito cardiaco e l’altro.
I sintomi della cardiomiopatia restrittiva si manifestano con un senso di stanchezza, gonfiore a mani e piedi e difficoltà a respirare quando si è sotto sforzo. E’ la forma di cardiomiopatia più comune tra gli anziani, e talvolta è provocata da altri disturbi.
